Is there and Outlook 15.4 uninstaller?
I removed Outlook.app from my computer.
Then I followed the steps you outlined above.
Then I imported an older Office 2011 profile which took hours.
Then I let it sit overnight.
Then I restarted my computer.
When I logged back in and opened Outlook, it asked me to Activate again.
